DUTIES AND RESPONSIBILITIES 1. To manage day-to-day operations of the Health Care Clinic. 2. To maintain health records, documenting each patient contact and updating patient profiles. 3. To check equipment and supplies, restocking as necessary. Check emergency kit. 4. To ensure confidentiality during visits and telephone contact. 5. To provide primary care for eligible patients health concerns during scheduled health service hours. 6. To distribute non-prescription medication after appropriate assessment. 7. To encourage health maintenance and promotion through counseling and awareness activities. 8. To take initial and interim history and vital signs on each patient. 9. To assist physician as needed during exams. 10. To assure that patients understand instructions. 11. Day 1 October 4, 2011 Tuesday 8:00am-4:00pm I visited her around 11:00am- 12:30pm  I ask for permission and explain the purpose of visit and then a moment ago she says its fine with her..Then I interview her to get her personal data and as a Clinic Nurse what is her responsibilities inside the clinic. Then afterwards a short interview, I sit there and give her a break to proceed on at the same time I’m observing. NOTE on 1st visit: On the first day I can only see 3 areas of responsibilities she does because I interview, though she a little bit busy at that time she still manage to organize her work load to facilitate

patient care and at the same time interviewing her. I just observed that the usual thing she do there is that she is taking the blood pressure of the patients going there, taking their height and weight especially the children, measures the blood sugar and responds to the other health needs of the people in their clinic and document all visits and health care outcome of the patients.

Day 2 October 14, 2011 Friday 8:00am-4:00pm I visited her around 11:00am- 12:30pm  On this 2nd visit of mine, well she still manages to chat with me for a while then she just let me conduct the observation. NOTE on the 2nd visit: and like the 1st visit she is maintaining those duties and responsibilities in the clinic, take those baseline data such as vital signs, height and weight. Assist with the doctors as necessary. Interview the patient and prioritize the plan of care to be render to the patient. Explaining the doctor’s order with them and assuring that those order are well understood. She secure those specimen that’s need to be send in the laboratory. Their some point that the doctor calls her and asks about what she thinks about client was experiencing. She also said that after of her duty she clean the clinic and checks each of the equipment functioning.

Day 3 October 17, 2011 Monday 8:00am-4:00pm I visited her around 11:00am- 4:00pm  Like the 1st and 2nd visit. Well a chat and observation was done. This time I have a long time to observe and chat with her. Note on the 3rd visit: Like the first and second visit she still managing the clinic complying with the duties and responsibilities she accepted. In addition, she confess to me that they were accepting phone calls in assisting the patients and they also evaluate the patients respond to the care given to their patient through phone calls and scheduled follow up check-up. The documents was also keep in the safe place and they keep its confidentiality.

REMARKS: Clinic health nurse is the practice of promoting and protecting the health of each individual: using knowledge, skill and attitude of true nursing practice, social and public health sciences a nurse in the clinic, community, and hospital should how to act independently with confidence that he/she is effective and efficient enough in rendering a quality care to every individual, family, or community in addition, the clinic nurse can apply her knowledge of strategies to choose the intervention(s) that meets the needs of each individual, family and thru community.
