Self-monitoring of oral anticoagulation: systematic review and meta-analysis of individual patient data Heneghan, C et al. The authors concluded that self-monitoring and self-management of oral coagulation is a safe option for suitable patients of all ages. Joseph Rowntree Foundation Dorset and Salford were identified as areas where structures and processes encourage and facilitate the involvement of older people in local decision-making. The research examined how older people in these areas were involved in commissioning and service delivery; what difference their involvement made; the factors which lead to successful engagement; and the barriers to involving older people. http://www.jrf.org.uk/publications/involving-older-people-more-power-to-their-elbow Person-based Resource Allocation: New approaches to estimating commissioning budgets for GP practices Nuffield Trust This document outlines Person-based Resource Allocation. It summarises the approach developed in a series of analyses over time. The analysis was funded by the Department of Health and the resulting formula was used within the ‘fair shares toolkit’ for allocations from primary care trusts (PCTs) to practices for commissioning, for 2010/11 and 2011/12. Project work included testing service models, reviewing registers and developing tools to improve and monitor quality. The Managing COPD as a long term condition workstream focused on supported self-care and regular review, reducing emergency admissions and improving medicines management. University of York This paper models individual expenditure using diagnostic information from previous hospital spells, plus a large set of attributed variables measuring population, general practice, and local hospital characteristics. It describes a procedure for setting budgets for general practices which reduces the problem of the lags in the available data.
