About gastritis Gastritis is a broad term for inflammation of the stomach lining, also called the gastric mucosa. This condition can be caused by many factors and, in some cases, may lead to an ulcer. For that reason, many of the same nutrients, herbs, and lifestyle changes for a peptic ulcer might also help someone with gastritis. Bacterial infection, most notably with Helicobacter pylori,1 is a major cause of gastritis. H. pylori is the same bacterium responsible for most cases of peptic ulcer. When considering treatments for gastritis, many researchers now look for substances that eradicate H. pylori, including bismuth2 and antibiotics.3 Other causes of gastritis include intake of caustic poisons, alcohol, and some medications (such as aspirin or adrenal corticosteroids), as well as physical stress from the flu, major surgery, severe burns, or injuries. For some people, a drug allergy or food poisoning can cause gastritis. Atrophic gastritis is a form of gastritis found particularly in the elderly, where stomach cells are destroyed, potentially leading to pernicious anemia. Product ratings for gastritis Science Ratings Nutritional Supplements Gamma oryzanol NAC Vitamin C Arginine Beta-carotene Glutamine Vitamin A Zinc Herbs

Chamomile Goldenseal Licorice Marshmallow Slippery elm Wood betony Reliable and relatively consistent scientific data showing a substantial health benefit. Contradictory, insufficient, or preliminary studies suggesting a health benefit or minimal health benefit. For an herb, supported by traditional use but minimal or no scientific evidence. For a supplement, little scientific support and/or minimal health benefit. What are the symptoms? Acute gastritis is typically characterized by nonspecific abdominal pain. Sincegastritis often occurs in severely ill, hospitalized people, its symptoms may beeclipsed by other, more severe symptoms. Gastritis that is caused by H. pylorieventually leads to peptic ulcers, which are characterized by a dull ache in the upper abdomen that usually occurs two to three hours after a meal; the ache is typically relieved by eating. Dietary changes that may be helpful Salt can irritate the stomach lining. Some research suggests that eating salty foods increases the risk of developing a H. pylori infection.4 Researchers have speculated that increased salt intake may also increase the risk of other forms of gastritis.5 Doctors commonly suggest that people with gastritis avoid spicy foods. However, one study found that capsaicin, the pungent ingredient in cayenne or chilipepper, protected against aspirin-induced gastritis in healthy persons. Whenpeople ate chili pepper followed by 600 mg of aspirin, stomach injury was considerably less than in people who took only aspirin.6 The researchers of thisstudy speculate that chili pepper helps by increasing blood flow to the stomach.Capsaicin has also been shown to protect against alcohol-induced gastritis inrats,7 though this has yet to be tested in humans. Some researchers have suggested that food allergies or intolerance may cause

gastritis.8 In one double-blind trial, people with proven food sensitivities showed clear evidence of irritation of the stomach lining (including swelling, bleeding, and erosions) when given foods to which they were known to react.9 However, most of these people did not have abnormal results from standard bloodtests for allergies. People suspecting food sensitivities or allergies shouldconsider discussing an allergy elimination program with a healthcare professional.Caffeine found in coffee, black tea, green tea, some soft drinks, chocolate, and many medications increases stomach acid,10 as does decaffeinated coffee.11Avoiding these substances should therefore aid in the healing of gastritis. Lifestyle changes that may be helpful Gastritis is common among alcoholics.12 Both heavy smoking and excessive alcohol consumption are known causes of acute gastritis.13 While heavy alcohol intake is clearly damaging to the stomach lining, preliminary evidence suggeststhat moderate alcohol consumption (generally defined as two drinks per day in women or three drinks per day in men) may actually protect against the development of gastritis by facilitating the elimination of H. pylori.14 When alcohol is consumed in greater than moderate amounts, it causes a wide variety of health problems. Many medications, such as aspirin and non-steroidal anti-inflammatory drugs (NSAIDS, such as ibuprofen), can induce or aggravate stomach irritation.15 People with a history of gastritis should never take aspirin or related drugs without first discussing the matter with their doctor. Other therapies Acute gastritis caused by trauma, stress, or severe illness usually heals rapidly when the underlying cause is resolved. Nonsteroidal anti-inflammatory drugs (NSAIDs) and ethanol are common stomach tissue irritants and their use should be limited in people with gastritis. Vitamins that may be helpful When H. pylori causes gastritis, free radical levels rise in the stomach lining.16 These unstable molecules contribute to inflammation and damage to thestomach lining. Vitamin C, an antioxidant that helps quench free radical molecules, is low in the stomach juice of people with chronic gastritis. This deficiency may be the link between chronic gastritis and the increased risk of stomach cancer. When people with gastritis took 500 mg of vitamin C twice a day, vitamin C levels in their gastric juice rose, though not to normal levels.17 Inanother trial, vitamin C supplementation (5 grams per day divided into severaldoses for four weeks) appeared to eliminate H. pylori infection.18 While nodirect evidence proves that taking vitamin C reduces

gastritis symptoms, scientists widely believe that any agent capable of knocking out H. pylorishould help people with this condition. The results of several clinical trials suggest that gamma oryzanol supplementation can help people with gastritis and other gastrointestinal complaints. In one study, people with chronic gastritis were given 300 mg of gamma oryzanol per day.19 After two weeks, 23% of people taking gamma oryzanol reported that it was “extremely effective” and 55% rated it as “moderately effective.” Another study produced similar results: People with various types of gastritis received 300 mg of gamma oryzanol per day. After two weeks, more than 62% of those with superficial gastritis, more than 87% of those with atrophic gastritis, and all people with erosive gastritis experienced improvement. These results were confirmed in a large study involving approximately 2,000 people with various gastrointestinal complaints, including several forms of gastritis.20 Some of these people required as much as 600 mg per day for symptoms to improve. People with gastritis wishing to take gamma oryzanol for more than six months, or in amounts exceeding 300 mg per day, should first consult with a physician. Various amino acids have shown promise for people with gastritis. In a double-blind trial, taking 200 mg of cysteine four times daily provided significant benefit for people with bleeding gastritis caused by NSAIDs (such asaspirin).21 Cysteine is a sulfur-containing amino acid that stimulates healingof gastritis. In a preliminary trial, 1–4 grams per day of NAC (Nacetylcysteine) given to people with atrophic gastritis for four weeks appeared toincrease healing.22 Glutamine, another amino acid is a main energy source for cells in the stomach and supplementation may increase blood flow to this region.23 Patients in surgical intensive care units often develop gastrointestinal problems related to a glutamine deficiency.24 When burn victimswere supplemented with glutamine, they did not develop stress ulcers, even afterseveral operations.25 Nevertheless, it remains unclear to what extent glutaminesupplementation might prevent or help existing gastritis. Preliminary evidence suggests the amino acid arginine may both protect the stomach and increase its blood flow,26 but research has yet to investigate the effects of arginine supplementation in people with gastritis. The antioxidant beta-carotene may reduce free radical damage in the stomach,27 and eating foods high in beta-carotene has been linked to a decreased risk of developing chronic atrophic gastritis.28 Moreover, people with active gastritis have been reported to have low levels of beta-carotene in their stomachs.29 In a preliminary trial, giving 30,000 IU of beta-carotene per day to people with ulcers or gastritis led to the disappearance of gastric erosions.30 In another

study, combining vitamin C and beta-carotene also led to improvement in most people with chronic atrophic gastritis.31 Zinc and vitamin A, nutrients that aid in healing, are commonly used to help people with peptic ulcers. For example, the ulcers of people taking 50 mg of zinc three times per day healed three times faster than those of people who took placebo.32 Since some types of gastritis can progress to peptic ulcer, it is possible that taking it may be useful. Nevertheless, the research does not yet show that zinc specifically helps people with gastritis. The amount of zinc usedin this study is very high compared with what most people take (15–40 mg per Day). Even at these lower levels, it is necessary to take 1–3 mg of copper per day to avoid a zinc-induced copper deficiency. People with ulcers who took 50,000 IU of vitamin A three times a day experienced a significant decrease in both ulcer size and pain.33 Because this amount of vitamin A is very high and can be quite toxic, usage requires the guidance of a doctor. A safe amount for women of childbearing age is 10,000 IU per day and probably 25,000 IU for other adults. In other preliminary research, using vitamin A together with drugs and proper nutrition eliminated erosive gastritis after three weeks in about 75% of affected people.34 Research has not yet shown that vitamin A supplementation specifically helps people with gastritis. People with pernicious anemia due to atrophic gastritis require very high amounts of vitamin B12. Are there any side effects or interactions? Refer to the individual supplement for information about any side effects or interactions. Herbs that may be helpful Many of the same herbs that are helpful for peptic ulcers may also aid people with gastritis. Licorice root, for example, has been traditionally used to soothe inflammation and injury in the stomach. Its flavonoid constituents have been found to stall the growth of H. pylori in test tube studies.35 However, there have been no clinical trials using licorice to treat gastritis. To avoid potential side effects, such as increasing blood pressure and water weight gain, many physicians recommend deglycyrrhizinated licorice (DGL). This form of licorice retains its healing qualities by removing the glycyrrhizin that causes problems in some people. Goldenseal is regarded as an herbal antibiotic and has been traditionally used

for infections of the mucous membranes. While no specific research points to goldenseal as a treatment for gastritis, there is some evidence from test tube studies that berberine, an active ingredient in goldenseal, slows growth of H. pylori.36 Modern herbal practitioners now prefer alternatives to goldenseal, since the plant is threatened with extinction due to overharvesting. Chamomile, high in the flavonoid apigenin, may soothe injured and inflamed mucous membranes. In addition, a test tube study has shown that apigenin inhibits H. pylori,37 and chamazulene, another active ingredient in chamomile, reduces free radical activity,38 both potential advantages for people with gastritis. Human clinical trials are needed to confirm chamomile’s effectiveness for treating gastritis. Demulcent herbs, such as marshmallow, slippery elm, and bladderwrack, are high in mucilage. Mucilage might be advantageous for people with gastritis because its slippery nature soothes irritated mucus membranes of the digestive tract. Marshmallow is used for mild inflammation of the gastric mucosa.39 Wood betony(Stachys betonica) has been used in European traditional herbal medicine for the treatment of heartburn and gastritis. Are there any side effects or interactions? Refer to the individual herb for information about any side effects or interactions.

Introductory Rites Greeting Penitential Rite Gloria Opening Prayer Liturgy of the Word First Reading: Proverbs 31:10-13, 19-20, 30-31 The woman who fears the Lord is to be praised. A reading from the Book of Proverbs 31:10-13, 19-20, 30-31 When one finds a worthy wife, her value is far beyond pearls. Her husband, entrusting his heart to her, has an unfailing prize. She brings him good, and not evil, all the days of her life. She obtains wool and flax and makes cloth with skillful hands.

She puts her hands to the distaff, and her fingers ply the spindle. She reaches out her hands to the poor, and extends her arms to the needy. Charm is deceptive and beauty fleeting, the woman who fears the Lord is to be praised. Give her a reward of her labors, and let her works praise her at the city gates. Lector 1: “The word of the Lord.” Assembly: “Thanks be to God” Responsorial Psalm : Psalm 103 “Loving and forgiving are you Lord Slow to anger, rich in kindness, loving and forgiving are you.”

Second Reading: Ephesians 5:2,21-33 This is a great mystery, but I speak in reference to Christ and the Church. A reading from the Letter of Saint Paul to the Ephesians 5:2 , 21-33 Brothers and sisters: Live in love, as Christ loved us and handed himself over for us. Be subordinate to one another out of reverence for Christ. Wives should be subordinate to their husbands as to the Lord. For the husband is head of his wife just as Christ is head of the Church, he himself the savior of the body.

As the Church is subordinate to Christ, so wives should be subordinate to their husbands in everything. Husbands, love your wives, even as Christ loved the Church and handed himself over for her to sanctify her, cleansing her by the bath of water with the word, that he might present to himself the Church in splendor, without spot or wrinkle or any such thing, that she might be holy and without blemish. So also husbands should love their wives as their own bodies. He who loves his wife loves himself. For no one hates his own flesh but rather nourishes and cherishes it, even as Christ does the Church, because we are members of his Body. For this reason a man shall leave his father and his mother and be joined to his wife, and the two shall become one flesh. This is a great mystery, but I speak in reference to Christ and the Church. In any case, each one of you should love his wife as himself, and the wife should respect her husband. Lector 2: “The word of the Lord.” Assembly: “Thanks be to God.” Gospel Acclamation “Alleluia” (Please stand and join the cantor on the refrain) Gospel All Respond: “ Praise to you Lord Jesus Christ.” Homily

There are many reasons to not buy a home: [By the way, I also put this in the category of Advice I want to tell my daughters, including my other article: 10 reasons not to send your kids to college.] Financial: A) Cash Gone. You have to write a big fat check for a downpayment. “But its an investment,” you might say to me. Historically this isn’t true. Housing returned 0.4% per year from from 1890 to 2004. And that’s just housing prices. It forgets all the other stuff I’m going to mention below. Suffice to say, when you write that check, you’re never going to see that money again. Because even when you sell the house later you’re just going to take that money and put it into another downpayment. So if you buy a $400,000 home, just say goodbye to $100,000 that you worked hard for. You can put a little sign on the front lawn: “$100,000 R.I.P.” B) Closing costs. I forget what they were the last two times I bought a house. But it was about another 2-3% out the window. Lawyers, title insurance, moving costs, antidepressant medicine. It adds up. 2-3%. C) Maintenance. No matter what, you’re going to fix things. Lots of things. In the lifespan of your house, everything is going to break. Thrice. Get down on your hands and knees and fix it! And then open up your checkbook again. Spend some more money. I rent. My dishwasher doesn’t work. I call the landlord and he fixes it. Or I buy a new one and deduct it from my rent. And some guy from Sears comes and installs it. I do nothing. The Sears repairman and my landlord work for me. D) Taxes. There’s this myth that you can deduct mortgage payment interest from your taxes. Whatever. That’s a microscopic dot on your tax returns. Whats worse is the taxes you pay. So your kids can get a great education. Whatever. E) You’re trapped. Lets spell out very clearly why the myth of homeownership became religion in the United States. Its because corporations didn’t want their employees to have many job choices. So they encouraged them to own homes. So they can’t move away and get new jobs. Job salaries is a function of supply and demand. If you can’t move, then your supply of jobs is low. You can’t argue the reverse, since new adults are always competing with you. F) Ugly. Saying “my house is an investment” forgets the fact that a house has all the qualities of the ugliest type of investment: Illiquidity. You can’t cash out whenever you want. High leverage. You have to borrow a lot of money in most cases. No diversification. For most people, a house is by far the largest part of their portfolio and greatly exceeds the 10% of net worth that any other investment should be.

Dear Ms. Orange: The latest denial letter for recipient Tony Ramirez is dated June 4, 2008 stating that a recipient is a member of a managed care plan and therefore no payments can be made unless the patient is disenrolled from SCAN. Patient had SCAN insurance but has exhausted benefits (please see attached SCAN/RMC denial letter) on October 25, 2006. In addition, patient has completely disenrolled effective 4/1/07 (please see attached disenrollment copy). There is no way that we can get a disenrollment earlier than 4/1/07 because patient already attempted to disenroll earlier than this date as early as February 2007 but was denied (attached copy). Just the same, should still be covered by Medi-Cal because patient exhausted the 100-day coverage as of November 18, 2006. A letter from EDS / CSU was received, dated 10/9/07 stating that my appeals were denied incorrectly by Appeals Unit, but there were no payments or adjustments made, except for a denial letter requesting for a HMO denial as mentioned earlier. Previous appeals for other dates of service were approved. Please refer to attachments with the document no. F2334486 for dates of service 11/22/06, 12/01/06, 12/16/06, 1/1/07 and 1/18/06, in reference to R.A. 08767115 dated 6/25/07. Please see attached new corrected denial letter from SCAN / RMC. Please re-consider payments of these claims and give us more information in regards to my inquiries. Attached are copies of all pertinent documentation like the remittance advice, original billing, all the appeals and CIFS submitted, a new appeal and corrected billing for the processing of the claim. We are hoping for a kind consideration on this matter. If you have further questions, you can contact me at 909-3704411 extension 204. Thank you.

Box 10229 Van Nuys, Ca 91410 RE: Loan no. 172443324 To Whom It May Concern: The purpose of this letter is to explain the unfortunate set of circumstances that have led to my mortgage delinquency. The type of loan that I have is thirty (30) years fixed jumbo interest only. The main reason that caused me to be late in payment is because of an injury that took place on August 11, 2008. I was on modified duty for three (3) months and was undergoing physical therapy. My hours were cut short and there were times that I was required to go home because there was no light duty available for me. I am slowly recovering and trying to get as much work hours as I could. This caused me to be further and further behind. I am not in the position to refinance due to loss of values in real estate market. I am already on regular schedule at the moment with occasional cancellations of duty. My months of hardship started from August 2008 onwards. I have existing debts on my credit cards and have been supporting my family members back in the Philippines as well. Lately, in January 2010, I was again advised to undergo a physical therapy plan, which I have strictly and effectively observed, the follow-up care as part of the treatment, weight –related measures and healthy eating habits. I was diagnosed with myofacial pain syndrome, lubrosacral radiculopathy, carpal tunnel syndrome, arthralgia and cervicalgia. Necessarily, it resulted in my my on-going financial stress, and affected my working schedules. I am however confident that sooner, I will be able to regain my normal physical health situation, which I assume to be only temporary. Please understand the circumstances leading up to my current standing are just too severe for me to uphold my financial commitment to you. I ask for your assistance in allowing the short sale of our property as the only remaining option so I can avoid foreclosure, save my credit and get back on my feet.

Sincerely and Respectfully, Marita Laguardia

DAN CARROLL, ESQ. Law of Offices of McCann and Carroll 2755 Jefferson St., Suite 211 Carlsbad, CA 92008 RE: Non-payment of Wanda Holderby’s medical services/Non-compliance with Medi-Cal requirements

Dear Sir: This is regarding the case of Wanda Holderby, a sub-acute patient admitted at our facility on June 18, 2005. She is in a semi-comatose state consequent to a series of strokes and is therefore unable to manage her financial resources. Her benefits exhausted from Blue Cross insurance on September 26,2005 which requires her to apply for Medi-Cal or pay the private rate of the facility.

Michael Taft is her grandson and only relative who handles her financial affairs. The patient was formerly Medi-Cal eligible with the share-of-cost of $990.00 last August 2005 and was approved by the Medi-Cal Riverside County. The requirements for annual re-determination needs to be submitted to reinstate Medi-Cal long-term care benefits as of September 2005 . The case was closed by the patient’s Medi-Cal Eligibility Worker for failure to turn in the necessary documents within the deadline. Therefore, a re-application needs to be submitted with the latest record of assets. Mr. Taft refused to apply for Medi-Cal at the time the benefits exhausted although he was informed that it was an urgent matter that needs to be resolved. Despite efforts of communicating with him, particularly trying to get in touch with him almost on a daily basis, he still declined to cooperate with the facility’s representatives. However, a Medi-Cal application was still submitted on October 10,2005 based on the facility’s available information. Nevertheless, the application was still denied because the documents secured from Riverside County were not sufficient to re-establish the eligibility since it would not provide the current income status of the patient. A case of abandonment has been filed against Mr. Taft through the Ombudsman. Mr. Taft was granted temporary conservatorship last December 21,2005 and was required to attend at a hearing in court last January 27,2006. The copies of the patient’s Medi-Cal documents including the latest application submitted were sent to him on January 5, 2006 for the reason that he may want to appeal the denial or start a process of a new application. The facility gave him numerous deadlines, and on his last conversation with Mr. Fred Frank (CEO) on January 25, 2006, he gave Mr.Taft a final notice of up to January 27, 2005 to apply for Medi-Cal in behalf of the patient. He has not made any efforts of appealing the case up to present time.

The outstanding balance of the patient has accumulated to the amount of $ 44,800.00. The patient owns a house in Riverside and is being maintained by Mr.Taft. His current address is 11829 National Drive, Riverside CA,92503 and his contact numbers are 951-6871282 and 7149815615. Attached herewith are the documents that might help resolve the case. If you have further questions please call me at 909-3704411 extension 204 or Mr. Frank at extension 202. Thank you.

April 26, 2006 DAN CARROLL, ESQ. Law of Offices of Mc Cann and Carroll 2755 Jefferson St., Suite 211 Carlsbad, CA 92008 RE: Norberto Rojo

Dear Sir: Please review the case of Norberto Rojo, a patient in our facility. Initial admission was on November 1, 2005. He is currently admitted under the sub-acute unit. His son, Alex Rojo has submitted an application to the Medi-cal Riverside County for the patient last November 14, 2005 (Eligibility Worker – Ms. Murillo 951-9406600) but was disapproved for having a bank deposit exceeding the limit of the Medi-Cal income guidelines.

He has applied for conservatorship in order to spend down the amount estimating to about thirteen thousand dollars ($13,000) and to be able to re-submit an application to Medi-Cal hoping for a
re-consideration. He was in a court hearing last April 24, 2006, and has informed our office on the same day that the hearing was moved to June 12, 2006 for the reason that the court needed appraisal information on the house that the patient owns. A soon as we knew that the patient owns a house, we assumed that he would not be approved Medi-cal if he applied again, even after spending down the amount in the bank. We intend to put a lien on the property. The patient’s outstanding balance is approximately $55,000.00. Alex Rojo’s address is 436 Pecan Rd., Phoenix, AZ 85040, his contact no.is 602-3914341. His legal representative is Attorney Albert Johnson Jr. and his office is located at 4166 Almond St., Riverside, CA and contact number of 951-6831777. May 19, 2006 Attention: Ms. Felonise De Bose-Ayodele Correspondence Specialist EDS P.O. Box 13029 Sacramento, CA 95813-4029 Dear Ms.De Bose-Ayodele, I would like to request a review of the denial of an appeal for Rigoberto Flores, for the date of service 6/22 – 6/30/04. The latest denial was received on 1/11/06 stating that information on the claim does not match the accommodation code in the TAR. The patient was sub-acute (non-ventilator) and an accommodation code of 76 was used in the claim, which matches the approved sub-acute E-TAR. I spoke to Judy Humphrey (415-9049608), nursing supervisor at San Francisco Medi-Cal Field office. According to her, the TAR has appropriate codings. The E-TAR does not use an accommodation code but only a service code of 11 for sub-acute. Please take back the payment of $2,574.79 and pay the correct amount of $3433.05. An amount of $858.26 was deducted with an adjustment code of 0475. I sent an appeal on 11/10/04 and received a denial on 1/17/05 stating an overlap payment for the dates of service 6/21-6/30/04. I waited for a response/denial for overlap billing with St. Bernardine’s before billing the dates of service 6/22 – 6/30/04 and used 04 as valid limit exception code. Attached is a copy of a letter from you stating that the denial was incorrect. There has been no response or adjustment to that claim for 45 working days. Please re-consider the payment of $3,433.05 for the dates of service 6/22- 6/30/04. Thank you. Sincerely, Anne Bolivar Medical Biller

Summary of Events: Twyla Harris-Mallard has been a sub-acute patient here in our facility since her admission in November 3, 2003. Paula Odume, the daughter and responsible party has not paid the Medi-Cal share-of-cost obligation amounting to $1305/month since January 2005 (which changed to $1345 in 2006). Reche Canyon Rehabilitation has already been assigned as payee by the Social Security starting December 2005. The benefits is amounting to $496. The facility requested to be representative payee for the benefits the patient is receiving through her deceased husband (Eldridge Mallard) from the L.A.Retirement System last January 2006, but they advised the facility that their representative payee program is under review. Initially, the facility requested to put the payments directing to Paula Odume on hold which was granted promptly. According to Mike Zamora (legal aid in 2005) the only way to get the back payment is to apply for conservatorship / public guardianship or a letter from Paula Odume (daughter and current representative payee) stating that she’s assigning us as the new representative payee. Paula Odume finally surfaced in June 2006 and cooperated by sending a letter to the retirement company requesting Reche Canyon Rehab as the new representative payee. The L.A. Retirement System as per their representative, Leonard Torres, did not approve the request stating that they have suspended their representative payee program, which prohibits them to assign new payees. The only solution that they suggest, to be able to get the back payment, is for the facility to apply for conservatorship or public guardianship.

A notice of action requesting current financial information such as insurance, bank statements, stocks, or property of any kind, was sent to the Mr. Howe’s brother, Ken Brown on August 10, 2006. He is the payee of Mr. Howe’s Social Security benefits as well. Medi-Cal investigation reveals that Mr. Brown received Social Security benefits in bulk last July as retroactive payment. The eligibility worker needed proof if the benefits were spent towards the patient’s medical care. Mr. Brown was constantly reminded by the facility about his obligation but to no avail. Failure to submit the documents on time led to the discontinuance of benefits effective October 2006 stated on a notice of action dated September 6, 2006. The facility’s request for an extension of deadline and was approved until the last week of October 2006, but Mr. Brown refused to cooperate and declined to coordinate with the patient’s eligibility worker. RE: George Bushnell Mr. George Bushnell was admitted here at Reche Canyon Rehabilitation on December 27, 2006 with Kaiser insurance. Since his Kaiser benefits did not cover the dates of service from January 1 to March 19, 2007 and from March 24, 2007 to May 4, 2007, he was billed with a private rate for his room and board, skilled nursing care and charged with miscellaneous bills for therapy, medications and supplies. Application for Medi-Cal / Medicaid benefits is highly recommended and the facility has to be provided with the copy of the duly accomplished application form as well as all pertinent documentation submitted. The facility should be updated with any developments pertaining to the upcoming pending application. Nevertheless, the patient will still be obliged to pay all the charges incurred during his entire stay in the facility until he obtains Medi-Cal long-term care benefits retroactively. Attached herewith is the statement of account that reflects the outstanding balance, which should be settled immediately. Dear Madam: A denial was received on RA# 10432766 dated Jan. 27, 2009 stating a 0069 denial code (duplicate from a previous adjustment). My request is based on adjustment made on Sept.26, 2008 with RA# 10084971 for dates of service 10/16–10/31/08. An amount of 4639.30 was paid instead of the requested amount of 5888.32. This is an adjustment error. Share-of-cost was already deducted on 10/1-10/15/08 (RA#0964588 dated 5/12/08) and on 10/16/08-10/31/08 dates of service (on RA#09708800 dated 6/3/08). Please correct this error by taking back the paid amount of 4639.30 and re-consider payment of 5888.32 for dates of service 10/16-10/31/08. Embutido Ingredients:
• • • 1 lb. ground pork 1/2 cup finely chopped carrots 1 cup (6 slices) finely chopped (sweet or cooked) ham

• • • • • • • • • • • • •

3 tbsp. minced green bell pepper 3 tbsp. minced red bell pepper 1/3 cup sweet pickle relish 1/4 cup raisins 3 whole eggs 1/2 cup grated cheddar cheese dash of liquid seasoning salt & pepper, to taste 1 tbsp. cornstarch slices (wedges) of hard-cooked eggs slices (wedges) of Vienna sausage aluminum foil, 10″ x 12″ sizes

Embutido Cooking Instructions: Prepare a steamer and set aside. Alternatively, prepare a baking pan and a wire rack and preheat oven to 350°F. In a bowl, combine all the ingredients and mix until well blended. Divide the mixture into 2 to 4 portions (depending on how many you want to make). Spread and flatten the mixture onto the center of each foil, divide the slices of hard cook eggs and Vienna sausages. Place each slices at the center of each mixture. Hold the foil onto your hand and roll until the ends of the mixture covers the eggs and sausages. Alternatively, by holding each ends of the foil, roll the mixture back and forth until it covers the slices of eggs and sausages in the center. Finally, roll the aluminum foil into a tightly packed log about 1″ to 2″ in diameter, sealing on both ends. Repeat with the remaining pork mixture. Place the embutido in a steamer and steam for an hour. Alternatively, place embutido in a wire rack on a baking pan, half filled with hot water. Cover with aluminum foil (Be sure the steam will not escape). Steam-bake in the center of the oven for an hour. Remove from the oven. Let it cool and slice into rings. Serve with your favorite catsup or sauces. Refrigerate unused embutido.

January 6, 2011 HOME LOAN TEAM BAC Home Loans Servicing, LP 4500 Amon Carter Blvd. Fort Worth, TX 76155 Investor Loan # 17244324 Gentlemen: I am highly grateful to learn of the approval of my application for the Home Affordable Modification Program concerning my home loan. In the same vein, I felt elated with your adjustment of a reasonable interest rate from a high 6.875% to as low as 4.625% for a period of time, extension of the loan term and “write off” or waiver of pre-paid late fees. I am also in accord with the servicing expenses paid to third parties in the amount of $1,635.65 and taxes and insurance in the amount of $7,897.00 which has been added to my new loan balance. But what disturb me most however is the modified monthly mortgage payments of $3,220.80 which obviously do not meet the salient features and purposes of the Home Affordable Modification Program. The “Clarity Commitment” significantly says that “ The loan modification is intended to help with your current needs and to put you in a better position to meet your loan obligations.” I was also made to understand that the program is envisioned to help bring or keep the loan current to an affordable and sustainable monthly payments in order to lessen the burden of homeowners because of severe financial hardships. In my letter to the Home Retention Group, I was emphatic in my request, among others, for an affordable and sustainable modified monthly payment of $2,000.00 or even a little more for a certain

period of time. It has to be so because of unforeseen financial hardships which was beyond my control. It arose out of an injury that happened on 11th August 2008. I had to undergo consecutive therapy sessions and could hardly attend to my modified hour duties for several months. The ailment certainly made it difficult for me to pay the monthly installments with the bank and credit facilities on time. Even my parents who are my dependents felt and suffered highly negative financial, if not emotional impact. Then, the lingering global recession was also one of the factors which contributed to my financial hardships. Again in January, 2010, I was again advised to undergo physical therapy plan. But because I strictly and efficiently observed the follow-up care as part of the treatment, weight-related measures and healthy eating habits I was able to regain a state of good health. Thereafter, I had to work double my time in different facilities in order for me to be able to cope up with the monthly dues such as utilities , car payments, credit cards, medical bills, insurance, winter and summer heating or cooling bills, not to mention the amount for support of dependents, my father and mother. Necessarily, shortly before and during the 3-month trial period, my monthly income increased but it was only temporary.

It appears that the monthly payment of $3,220.80 was erroneously based on my current monthly gross income immediately before and during the 3-month trial period. In this regard, the Federal government envisions the monthly payments to be not less than 30% of the net income and not the gross monthly take. Besides, under the Obama Administration Affordable Plan, lenders will be able to bring down monthly payments by reducing the principal loan to approximate and reflect the current market value. The past due interest of $59,365.90 should not have been added to the principal loan but instead included in the unpaid late fees. In Southern California, the home value dropped an average of 20% to 30%. The loan value of the house now is $ 360,000,00. I now owe the bank almost double the worth of my house. In exploring options, lenders under the California Law, may modify re-write, re-evaluate the loan terms so that the monthly payments could be affordable, no matter how much is the income. For anyway, loan modification has no formal guidelines and therefore the agreement may vary subject to the ability to pay and the hardships and difficulties encountered by the homeowner. We have learned to like our house and for my sake and the bank as well, I have no plans to walk away as much as possible. Our concern now is to save the house where we are presently at peace. Our only request is for the lender to correct the terms and conditions of the agreement pursuant to No. 4 of the “Additional Agreement”, meaning, to at least reduce the monthly installment to an affordable level of $2,000.00 monthly or a little more and my principal loan for a minimum of five (5) years, for me to be able to come up with the budgeting of the entire household. I will be even more grateful for your favorable action to our request so that I can truly enjoy the benefits of the Home Affordable Program of the government. Truly yours, MARITA U. LAGUARDIA

My son came here to the U.S. when he was 6 years old. His Dad came first here for a year and was away from here for almost a year and a half because he was still petitioning us at that time. We lived in the Philippines with our parents. All of the family very well loved him, and at that time he was the only and first grandchild. When he first came here, he was in a way raised differently from what he expected. He always had differences with his father who was strict and disciplinarian in a way. Coming from the Phils., from a different culture made it a little difficult for him to adjust. He went to elementary school and was a very good student. We moved to Redlands from Colton about 4 years ago. He went to Clement Middle School on 6th grade but had a very

difficult time coping because occasional bullying. He was overweight at that time which I believe caused his bullying. He tried to fit in by joining the band and played the clarinet. I had to move him to Packinghouse Christian Academy because he wasn’t happy with Clement because of the school environment and it was already affecting his grades. I believe in Packinghouse school’s good environment and education based on Christian teaching will be good for him. The two years of middle school that he stayed at Packinghouse was good. He also has this medical condition called Acid Reflux which is aggravated by stress apart from stomach acidity. He would often regurgitate (throw up) occasionally if he eats fatty or spicy food. This condition started when he was around 12 y.o. His condition is regulated by medication but stress factors often contribute to the severance of the symptoms. For his highschool freshman year, he wanted to go to a bigger school hoping for bigger opportunities. He was in Cross-Country for the whole semester. After a few weeks in school, I noticed that he wasn’t happy. There were episodes of ups and downs of emotions, and he has a different attitude with us, his parents. He seemed very stressed out at times but pretends that he can handle things. There were two times that I saw bruises I his upper chest and hands and that was the time that I decided to pull him out of the school. He has been in fights and when I confronted him about it last December 2010, he gave me the worst attitude and ran away. We looked for him around the community with the help of some friends and reported it to the police. After four hours, he called us saying he was in a gas station in Loma Linda. We picked him up and discussed why he had to run away. He didn’t want to discuss what was happening at Citrus Valley School at that time and I believe there is certain stress and pressure. That was the time I decided to bring him back here at Packinghouse Christian Academy. He has been telling me before that he seeks to talk to a Pastor whom he could share his feelings and struggles in life.

Psalm 86:11 Teach me Your way, O LORD; I will walk in Your truth; Unite my heart to fear Your name. January 31st, 2011 by Pastor Ed in devotional David prays his desire to remain teachable, flexible and adaptable to God’s plans while he was passing through difficulties in his life. God will use whatever hardship we are going through for His greater purposes in our life also if we let Him also. We notice that David also reminds himself not to be too wise in his own eyes in this prayer. The opposite of an united heart is a divided heart which is a heart caught

between two possibilities. Could David be admitting that one part of his heart really does seek to honor God while the other part seeks to rebel? We believe that is why David prays for God to ‘unite’ and give him an undivided, single-heartedly loyalty to his God, When Jesus was asked what was the greatest commandment he replied, “You shall love the LORD your God with all of your heart, with all of your soul and with all of your the mind and strength.’ Total love and devotion is exactly what God desires and looks for in us. Wealthy and successful, Alfred Nobel was an engineer, businessman, and inventor. One morning, Nobel picked up a newspaper and, to his surprise, read about his own death. Obviously, the editor of the paper had made a mistake. As Nobel read his own epitaph, however, he was appalled to see his life summed up with the phrase, ‘merchant of death’ because at that point in his life, Alfred Nobel was known as the inventor of dynamite. He determined that day, that he would work to be remembered differently. So, he devoted his energies from that day forward to the creation of an annual prize to be given to the ones “who during the preceding year, shall have conferred the greatest benefit to mankind.” We know that award today as the Nobel Prize. Nobel later wrote, “Every person ought to have the opportunity to correct his or her epitaph in midstream.”

“LORD, please use us to write a good epitaph for our own life this day.”

Psalm 93:4 The LORD on high is mightier than the noise of many waters, Than the mighty waves of the sea.
February 7th, 2011 by Pastor Ed in devotional God’s strength and might greatly exceeds any pounding surf or tsunami. The many storms of life by their sheer power will sometimes throw a person off into thinking otherwise. We struggle and get our eyes off the Lord and onto the waves crashing over the sides of our boat. It is then that we need a quiet moment to reflect on the great truth in this verse. The power of crashing waves or a cascading waterfall is nothing compared to the power of the King. Iguazu falls on the border of Brazil and Argentina is a spectacular waterfall system of 275 separate falls along the 2.7 km (1.67 miles) of the Iguazu River. Carved into a wall on the Brazilian side of the falls are the words of Psalm 93:4, “Mightier than the thunders of many waters, mightier than the waves of the sea, the Lord on high is mighty!” (RSV) Below that Scripture some wise believer chiseled these additional words, “God is always greater than all of our troubles.” It does not matter how high the floods of life come up over our head, the Lord of all creation remains greater above them all. “You O LORD are our strength and protection on the road of life this day. We gladly submit to Your Lordship over every detail no matter how small that You desire to lead us in.”

The Psalms 37
The Insecurity of the Wicked
A Psalm of David.

1 Fret not thyself because of evildoers, neither be thou envious against the workers of iniquity. 2 For they shall soon be cut down like the grass,

and wither as the green herb. 3 Trust in the LORD, and do good; so shalt thou dwell in the land, and verily thou shalt be fed. 4 Delight thyself also in the LORD; and he shall give thee the desires of thine heart. 5 Commit thy way unto the LORD; trust also in him; and he shall bring it to pass. 6 And he shall bring forth thy righteousness as the light, and thy judgment as the noonday. 7 Rest in the LORD, and wait patiently for him: fret not thyself because of him who prospereth in his way, because of the man who bringeth wicked devices to pass. 8 Cease from anger, and forsake wrath: fret not thyself in any wise to do evil. 9 For evildoers shall be cut off: but those that wait upon the LORD, they shall inherit the earth. 10 For yet a little while, and the wicked shall not be: yea, thou shalt diligently consider his place, and it shall not be. 11 But the meek shall inherit the earth; Mt. 5.5 and shall delight themselves in the abundance of peace. 12 The wicked plotteth against the just,

and gnasheth upon him with his teeth. 13 The Lord shall laugh at him: for he seeth that his day is coming. 14 The wicked have drawn out the sword, and have bent their bow, to cast down the poor and needy, and to slay such as be of upright conversation. 15 Their sword shall enter into their own heart, and their bows shall be broken. 16 A little that a righteous man hath is better than the riches of many wicked. 17 For the arms of the wicked shall be broken: but the LORD upholdeth the righteous. 18 The LORD knoweth the days of the upright: and their inheritance shall be for ever. 19 They shall not be ashamed in the evil time: and in the days of famine they shall be satisfied. 20 But the wicked shall perish, and the enemies of the LORD shall be as the fat of lambs: they shall consume; into smoke shall they consume away. 21 The wicked borroweth, and payeth not again: but the righteous showeth mercy, and giveth.

22 For such as be blessed of him shall inherit the earth; and they that be cursed of him shall be cut off. 23 The steps of a good man are ordered by the LORD: and he delighteth in his way. 24 Though he fall, he shall not be utterly cast down: for the LORD upholdeth him with his hand. 25 I have been young, and now am old; yet have I not seen the righteous forsaken, nor his seed begging bread. 26 He is ever merciful, and lendeth; and his seed is blessed. 27 Depart from evil, and do good; and dwell for evermore. 28 For the LORD loveth judgment, and forsaketh not his saints; they are preserved for ever: but the seed of the wicked shall be cut off. 29 The righteous shall inherit the land, and dwell therein for ever. 30 The mouth of the righteous speaketh wisdom, and his tongue talketh of judgment. 31 The law of his God is in his heart;

none of his steps shall slide. 32 The wicked watcheth the righteous, and seeketh to slay him. 33 The LORD will not leave him in his hand, nor condemn him when he is judged. 34 Wait on the LORD, and keep his way, and he shall exalt thee to inherit the land: when the wicked are cut off, thou shalt see it. 35 I have seen the wicked in great power, and spreading himself like a green bay tree. 36 Yet he passed away, and, lo, he was not: yea, I sought him, but he could not be found. 37 Mark the perfect man, and behold the upright: for the end of that man is peace. 38 But the transgressors shall be destroyed together: the end of the wicked shall be cut off. 39 But the salvation of the righteous is of the LORD: he is their strength in the time of trouble. 40 And the LORD shall help them, and deliver them: he shall deliver them from the wicked, and save them, because they trust in him.
